Request a copy of your medical record or authorize release to another provider.
To request your medical records, please complete the online request form below. Requests for personal copies may include a copying fee. Most requests are processed within one business week.

Helpful Tips
- Have your full name and date of birth available.
- Be specific about what you need (dates of service, visit notes, lab results, imaging reports, etc.).
- If sending to another provider, include their contact information.
